Sư Bà Diệu Tâm thế danh Văn Thị Mai, pháp danh Nguyên Từ, sinh năm Kỷ Mão (1939) tại Quảng Nam, Việt Nam, là đệ tử đầu của Sư Cụ thượng Đàm hạ Minh. Ông bà thân sinh của Sư Bà tuy theo tân học nhưng một mực thâm tín Phật Pháp. Ngay từ thuở nhỏ Sư Bà vẫn thường theo cha mẹ đi chùa tụng kinh lễ Phật. Năm 15 tuổi Sư Bà cùng song thân đến Chùa Sư Nữ Bảo Thắng ở Hội An cầu pháp với Sư Cụ thượng Đàm hạ Minh và xin thế phát xuất gia. Bốn năm sau, vào ngày 19.06.1959 (Kỷ Hợi), Sư Cụ cho Sư Bà thọ Sa Di Ni và hai năm sau đó, ngày 17.11.1961 (Tân Sửu) thọ Thức Xoa Ma Na Ni. Đến năm 1965, nhận thấy đạo hạnh đã đầy đủ, Sư Cụ cho Sư Bà Diệu Tâm thọ Tỳ Kheo Ni, Bồ Tát Giới tại Đại Giới Đàn Vạn Hạnh tổ chức ở Tổ đình Từ Hiếu, Huế (17.-18.07.1965 Ất Tỵ) do cố Đại Lão Hòa Thượng thượng Giác hạ Nhiên, đệ nhị Tăng Thống GHPGVNTN làm đàn đầu.
